Default smiters receive damage, how?

Ive been using 600/smite for some time and my friend asked me a question about it that i couldnt explain myself. Why does the smiter receive damage even though it is completely away from anything while in dungeons?

This.
And because of that, always cast retribution first.

yep, and try to have the 600 wand the RoD casters a bit, can help a little. I just allow for it in my smiters build and let them heal a little to counteract it.

As long as the smiter casts retribution first, should be able to eal through it easily using a signet heal.
